Lot LOT-0001 — HOSPITAL PHARMACY EQUIPMENT UPGRADE

The subject of the order is the HOSPITAL PHARMACY EQUIPMENT UPGRADE; AS PART OF THE PROJECT TITLED: "IMPROVED EFFICIENCY, ACCESSIBILITY AND QUALITY OF PROVIDED ONCOLOGY SERVICES BY THE J. K. ŁUKOWICZ SPECIALIST HOSPITAL IN CHOJNICE" IMPLEMENTED UNDER THE NATIONAL RECOVERY AND RESILIENCE PLAN: COMPONENT D "EFFICIENCY, ACCESSIBILITY AND QUALITY OF THE HEALTH CARE SYSTEM" Investment D1.1.1 "Development and modernization of infrastructure of high-specialty care centers and other medicinal entities", specified in detail in the Description of the Subject of the Order (OPZ) / Assortment and Price Form - Annex No. 2 to the SWZ and the Technical Parameters Form – Annex No. 3 to the SWZ.

Award Criteria:

• a. OFFER PRICE The price criterion will be considered based on the price provided in the offer. The price will be scored based on the formula: C= (lowest price x criterion weighting) / price of the evaluated offer The offer with the lowest price receives the maximum number of points, i.e., 60 points. (60)
• b. ORDER EXECUTION DEADLINE The criterion will be considered based on the Contractor's declaration submitted in Annex No. 1 to the SWZ – Offer Form regarding the delivery deadline. Points will be awarded in accordance with the points based on the formula:

   Points will be awarded in accordance with the points based on the formula:

• c. QUALITY ASSESSMENT Parameter assessed in accordance with Annex No. 3 to the SWZ − The criterion will be considered based on the Contractor's declaration submitted in Annex No. 3 to the SWZ (Technical Parameters Form). − The highest number of "small points" that a Contractor can receive in this criterion is 25 – this is the sum of "small points" awarded in accordance with Annex No. 3. OJ = (number of "small"* points awarded by the commission to the evaluated offer x criterion weighting) / max. number of small points
  • points awarded by the commission in accordance with the technical description of equipment form (offered parameters – maximum number of points the Contractor will receive: max 25 pts). The offer with the highest quality assessment receives the maximum number of points, i.e., 30 points. (30)
